The purpose of this project is to: 1) evaluate the Diabetes & Childhood Asthma Mobilization and Faithful Families Eating Smart and Moving More (FFESMM) grant projects; 2) provide technical assistance to grantees on an as-needed basis; 3) organize and analyze Chronic Disease Self Management Program (CDSMP) data; 4) support Capacity Building Workshops and analyze “pre” and “post” workshop survey data; and 5) provide strategic planning and convening support, as requested.
