CSHP is conducting an evaluation of the Delaware Passport to Independence program that assists nursing home residents in their transition back to the community. Through surveying clients, clients' families, assessment workers, and case managers the research team will identify the elements that facilitate a completed transition as well as impediments, and will assess the quality of life of those who have returned to the community, and whether or not their needs are being met.
