This project will provide independent evaluation research services focusing on selected components of the State Partnership Program. The evaluation aim is to assess the successes and barriers to implementation and gauge community stakeholder perceptions of their participation in Health NJ 2020. CSHP will conduct analysis and prepare a draft report of findings, documenting trends and patterns of adoption of best practices in data collection and reporting and other goals of the DHSS Disparities Plan following receipt of completed questionnaires.
