The scope of this proposal is to develop a self-assessment tool for the NJDHSS to collect programmatic data annually. The survey will be used to assess the extent that health disparities initiatives are making progress in reaching minorities statewide. The survey will be administered yearly for the length of the project (5 years) by the Office of Minority and Multicultural Health. Additionally, CSHP will evaluate survey data and prepare required reports.
