Evaluation of the Transition Process for Children with Special Health Needs
The medical transition process of children with special health care needs was identified by the Division of Family Health Services' (FHS) taskforce as an important area to examine as part of their required needs assessment activities. To provide FHS with information about these transitions in New Jersey, CSHP will conduct an in-depth analysis of the 2000-2002 SLAIT data, interviews with parents or guardians of these children, and interviews with physicians who treat children and adults with the following medical conditions: Down Syndrome, Cleft Lip and Palate, Spina Bifida, and Diabetes. Together these components will explore the transition process of children with special health care needs, their potential barriers, concerns, and unique issues.
