The purpose of this project is to establish a New Jersey hospital quality improvement initiative targeted to improve the treatment of hospitalized patients with heart failure. New Jersey hospitals facing challenges in improving heart failure quality indicators were invited to join this initiative. Through intensive technical assistance, the hospitals will target specific improvements that will be identified and tracked. This project is co-sponsored by the New Jersey Department of Health and Senior Services and is conducted in conjunction with the New Jersey Hospital Association Quality Institute.
