The Comprehensive Primary Care (CPC) initiative, funded by the Center for Medicare & Medicaid Innovation, is a multi-payer effort designed to foster collaborations between public (i.e., Medicare) and private health care payers to transform and improve primary care. In order to plan, assess, and provide technical support to the organizations participating in this initiative, the Center is guiding and facilitating a stable, sustainable multi-stakeholder environment conducive for pilot testing payment reform and other practice transformation efforts in New Jersey. Activities in this project include facilitating and organizing stakeholder meetings, stakeholder engagement and problem resolution, and contributing to learning network and strategic planning efforts.
