Advancing Development of Safety Net Accountable Care Organizations

This project is aiding in the development of safety net accountable care organizations in low-income communities across New Jersey. For each selected community, the project describes current health care utilization patterns, engages local stakeholders to identify care coordination opportunities, and projects potential cost savings through the application of care coordination improvement strategies.
