Dissemination of Learning from Adapting a High-Utilization Team Model
From 2012 to 2015, funded by a Health Care Innovation Award (HCIA) from the Centers for Medicare & Medicaid Services, Rutgers Center State Health Policy (CSHP) collaborated with Camden Coalition of Healthcare Providers (CCHP) and Center for Health Care Strategies (CHCS) to facilitate the adaptation of CCHP’s intensive care coordination model targeting super-utilizers (patients with complex physical, behavioral, and social needs) in four diverse communities. In the coming year, CSHP will compile the rich learning from the three years of model adaptation into the following products: 1) a data book/infographic describing the adaptation process, care strategies, and patient-reported outcomes; 2) a peer-reviewed journal article on the benchmarking analysis of hospital cost savings; and 3) a synthesis of patient and staff stories in a consolidated report featuring project-wide and cross-site perspectives suitable for nontechnical audiences. CSHP will also work with CCHP, CHCS, and the adaptation sites to disseminate these products widely to practitioners, policymakers, and other stakeholders interested in developing super-utilizer interventions.
