The Center is summarizing quantitative findings and preparing and disseminating a data brief of developed information on social and economic determinants of health for 13 New Jersey communities that are candidates for establishing Medicaid Accountable Care Organizations. The CSHP team is working with data to focus the publication on a limited number of measures that most effectively communicate variations in the social determinants of health and health care use in the 13 communities.
