Evaluation of HRSA Healthy Communities Access Program

CSHP is working in collaboration with NORC (National Opinion Research Center at the University of Chicago) in conducting an evaluation to measure the effectiveness of the HRSA-funded Community Access Program (CAP). CAP strives to improve access to care to low income and vulnerable populations in the forms of insurance expansion, systems integration leading to more provider efficiency, increased patient education and enrollment in programs, improved and integrated information systems, and expansion of outreach. The evaluation will include a provider survey, key informant interviews of the grantee coalitions, focus groups, and an analysis of the grantee reported six month program monitoring updates.
